A key area of focus for OPIP has been supporting primary care practices to be medical homes and provide robust and team-based care. Since 2010, at last one project in each year of OPIP’s work has been focused on enhancing medical homes for children and adolescents.  In Oregon, there is a priority around medical homes through the Patient Centered Primary Care Home (PCPCH) program and related incentive metrics and requirements for value-based payments to PCPCH sites.
