Throughout the SBHC Adolescent Well-Care project, OPIP conducted trainings and facilitation to the pilot SBHC sites on three CCO incentive metrics, specific to their adolescent populations. These metrics include: (a) adolescent well-care visits, (b) depression screening and follow-up, and (c) substance abuse screening, brief intervention, referral, and treatment (SBIRT).
